The following forms need to be completed, signed and on file in your child’s records at school. All forms are available in the school’s Front Office.
After School Classes Release and Waiver
Parents must sign this form if their child would like to participate in any of our after school extracurricular classes. (Form in PDF format)
After School Enrichment Program
Our school offers an excellent after school program for children of working parents. It is held in LA Casa and operated by LA Casa staff. (Program Form and Release Form in PDF format)

Emergency Medical Liability Waiver
In the event your child becomes ill or injured while under school authority, and the school is unable to reach parents or guardians, this form authorizes the school to arrange for emergency medical treatment for your child. Parents may also sign this form if they choose not to grant their permission. (Form in PDF format)
Enrollment Contract
Presented to you when your child has been accepted. Please have your signature witnessed when you sign the contract. Signed contract must be returned with $500.00 Advance Deposit by the deadline stated in the accompanying letter. Please note Middle and Upper School parents and students have additional contracts and forms to sign. These are available in the Front Office.

Field Trip Authorization
Parents must sign this form in order for your child to participate on school-sponsored field trips. (LA Casa – Toddler and Primary students do not usually participate on field trips.) (Form in PDF format)
Health Record
All new and returning students, regardless of age, must submit a Health Record completed by a physician before the first day of school.
La Casa – Toddler Program and Half-Day Primary students who stay for lunch must have the VI Dept. of Human Services three-part Health Record Form completed by a physician every year before the first day of school. This includes annual stool and urinalysis testing and an up-to-date immunization record. This three-part form is available in our Front Office on Mon.-Fri. from 8:00 am – 4:00 pm during August. Full Day Primary through Upper School students must have a one-part Health Record Form completed by a physician every year before the first day of school. (Form in PDF format.)
Medication Authorization
This form gives the school permission to administer over-the-counter medication to your child to help reduce fever, alleviate headache or any other pain/discomfort your child is experiencing at the time should it be deemed necessary after every attempt has been made to reach the parents or guardians. The medication would be either Children’s Tylenol or Advil (tablet or liquid). Parents may also sign this form if they choose not to give their permission. (Form in PDF format)
Photo Release
Throughout the school year, many opportunities arise to take photographs of VIMS students as they participate in classroom, cultural and sports activities both on- and off-campus. Signing this form allows your child’s photograph to be included in various VIMS publications. (Form in PDF format)
Registration Card
This card contains vital information about your child that we need in case of an emergency. Blank cards are available in the front office. Please keep it up-to-date.
School Bus Waiver of Liability
We are fortunate to have morning and afternoon school buses provided by the VI Department of Education. You may register your child for this in the Front Office. All students taking the school bus must have a liability waiver signed by their parents. (Form in PDF format)
Parents – Please remember that there is no bus service on days when VI Public Schools are closed (and VIMS is open).

Student Questionnaires
Parents are requested to complete the appropriate level questionnaire in order for us to better understand your child and his/her special needs. This should be completed and returned with the Enrollment Form and Application Fee.
